Parent Declaration Form

If you are entitled to funding, then in order to receive the funding, we must have a fully completed parent declaration form and an eligibility code before our termly cut off date.

If the form is not fully completed, then you risk the possibility of losing some of your funded hours and you will have to pay for the childcare yourself to keep your child’s place at pre-school.
